My HR File
Your own HR record in one place: leave taken this year, your most recent performance review, any open disciplinary notices, and a form to request time off.
Who can use it — Everyone in a standard (civil) workspace can open this page and see their own Leave tab — there's nothing to configure to request leave. The Performance Reviews and Disciplinary Notices tabs only appear if your role has been granted "View Performance Reviews" or "Manage Performance Reviews" (and the matching disciplinary permissions) — if you can't see one of these tabs but think you should, ask your admin. See Roles & Permissions. This page is not available in a standalone Trade Pack workspace — it's part of the civil HR module.

How to use it
The top of the page shows a row of tiles — tap any of them to jump straight to the matching tab:
- Annual Leave Used YTD and Sick Leave Used YTD — approved weekdays taken so far this calendar year. This is time used, not your remaining balance — contact HR/payroll for what's left.
- Last Review (if you have access) — the date of your most recent performance review, colour-coded: grey for Scheduled/Not yet, orange for Awaiting your acknowledgement, blue for Completed/Acknowledged.
- Open Notices (if you have access) — how many issued disciplinary notices are still waiting on your acknowledgement. Red if there's at least one, green if you're clear.
Below the tiles, the same three areas are laid out as tabs: Leave, Performance Reviews, Disciplinary Notices.
Leave
The Leave tab lists every leave request you've submitted — Dates, Type (Annual, Sick, RDO, Carer's, Long Service, Unpaid, Other), Status (Pending, Approved, Rejected), any Manager Notes left when it was decided, and whether you attached a document.
To request time off, click Request Leave (top of the table, or on the empty state if you've never submitted one). It opens a slide-over:
- Type — pick the leave type.
- Start Date / End Date — the end date can't be before the start date.
- Reason — optional free text.
- Medical Certificate / Document (under Evidence) — optional attachment, useful for sick leave. Accepts PDF, Word, JPG, PNG or WebP, up to 20MB.
Submitting creates the request with Pending status and notifies whoever manages leave for your team.
While a request is still Pending, click Open on its row to edit the type, dates, reason or attachment, or delete it outright if you no longer need it. Once it's been Approved or Rejected, it's locked — the decision and any manager note stay visible in the row.
Leave is approved or rejected by your team's approvers in Team Approvals. A full, team-wide view of leave requests lives in Leave Requests for those with the right permission.
Performance Reviews
If you have access, the Performance Reviews tab lists reviews on your file — Reviewer, review Period, Scheduled date, Status (Draft, Completed, Acknowledged), whether it's been Acknowledged, and the Next Review due date.
Click View on a review to open the full detail: reviewer, period, scheduled/completed/acknowledged dates, next review due date, any characteristic ratings (Excellent / Good / Satisfactory / Needs Improvement) your reviewer set, and free-text notes on goals achieved, goals for next period, and comments & suggestions.
If a completed review needs your sign-off and you haven't acknowledged it yet, the modal shows a banner explaining that acknowledging confirms you've read and understood it, and the button changes to Acknowledge. Use Download PDF on any row to save a copy.
Reviews themselves are scheduled and completed by managers in Employee Records — from here you can only view and acknowledge your own.
Disciplinary Notices
If you have access, the Disciplinary Notices tab lists any notices issued to you — Type (Verbal Warning, Written Warning, Final Warning, Termination), Incident Date, Issued Date, Status (Draft, Issued, Acknowledged, Closed), whether it's Acknowledged, and who Issued it.
Click View to see the full notice — including the description and any recorded outcome. If it's Issued and you haven't acknowledged it, a banner makes clear that acknowledging only confirms you've read and understood it — not that you agree with it — and the button changes to Acknowledge. Use Download PDF to save a copy.
Notices are issued by managers in Employee Records — from here you can only view and acknowledge your own.

Tips & common mistakes
- The leave tiles show days used, not your remaining entitlement — this page doesn't track balances, so ask HR/payroll if you need to know what's left.
- You can only edit or withdraw a leave request while it's Pending. Once it's Approved or Rejected, get in touch with whoever manages leave if it needs changing.
- No Performance Reviews or Disciplinary Notices tab? That's a permission setting, not a sign you have none on file — check with your admin.
- Acknowledging a disciplinary notice isn't agreeing with it — it just confirms you've read and understood it.
- Can't find this page at all? Standalone Trade Pack workspaces don't include the civil HR module, so My HR File won't appear.
